Authorization terms
I authorize 365 Restoration to enter the property and perform emergency mitigation services — water extraction, drying, board-up, stabilization, containment and cleaning — in accordance with IICRC S500, S520 and S700 standards.* I direct my insurance carrier to pay 365 Restoration directly for covered mitigation services, and authorize the release of claim information to the contractor. I permit photographs and moisture documentation of the loss to be used for claim documentation and, with identifying details removed, in before/after examples. 365 Restoration · VA Class A Contractor Lic. 2705138091 · services performed under industry standard of care. Charges not covered by insurance remain the responsibility of the property owner. 365 Restoration is a licensed contractor, not a public adjuster, and does not negotiate or settle your claim on your behalf.
Your right to cancel — Virginia
If this agreement is signed at your home or anywhere other than our regular place of business, you may cancel it without penalty or obligation within three business days of signing. To cancel, notify us in writing at our office address or by email before midnight of the third business day.
Written notice of cancellation may be sent to 365 Restoration, 1720 Lambert Court, Unit I, Chesapeake, Virginia 23320, or by email to steve@randrva.com.
This is an emergency. I request that 365 Restoration begin mitigation work immediately and I waive my three-business-day right of cancellation as to the emergency mitigation work performed at my request. This waiver does not apply to any reconstruction or repair work agreed to later. Optional. Leave this unchecked to keep your full three-business-day cancellation window; emergency work may then be scheduled after that period unless you request otherwise.
