Odor Treatment Safety Questionnaire & Consent (Ozone, Hydroxyl & Chemicals)

Smoke and stubborn odors are treated with ozone, hydroxyl generators, air scrubbing, fogging, antimicrobials or sealers — sometimes more than one. Each has its own safety rules: ozone needs an empty building, hydroxyl can usually run while you stay, and chemicals need drying and airing time. Answer the safety questions so we pick the right method for your home, then sign so we can schedule it.

Getting the area ready — owner / tenant checklist

Preparing and cleaning the area is the owner's or tenant's responsibility. If you would rather we do it, tell us and we will quote it as an added charge.

Remove the source of the odor first

  • Take out all trash, food waste, spoiled food and full recycling.
  • Empty and clean the refrigerator and freezer of anything spoiled; leave doors propped open if power is off.
  • Empty, clean and remove litter boxes, pet bedding, cages and pet waste from the yard by the door.
  • Remove ashtrays, cigarette butts, vape pods, candles and incense.
  • Bag and remove soiled clothing, bedding, towels and rags, or wash them before we arrive.
  • Clear out any wet, moldy or fire-damaged material that has already been released for disposal.

Clean the hard surfaces

  • Wash floors, counters, cabinet fronts, doors, trim and windowsills with a general cleaner.
  • Wipe down walls in the worst-smelling rooms — smoke and cooking film hold odor.
  • Clean sinks, tubs, toilets and drains; run water in unused drains so traps are not dry.
  • Vacuum carpets, rugs, upholstery and along baseboards; empty the vacuum outside afterward.
  • Launder or dry-clean curtains, slipcovers and washable soft goods if they can be done in time.

Clear and open the space

  • Pick up belongings off the floor so we can place equipment and reach every corner.
  • Open closet, cabinet, pantry and vanity doors; pull drawers out a few inches.
  • Move furniture a few inches off the walls where possible.
  • Remove clutter, boxes and stored items from the rooms being treated.

Take these items out of the treated area

  • People, pets, birds, fish, reptiles and live plants — out of the building for ozone.
  • All food, drinks, spices, pet food and medications — sealed or removed.
  • Rubber and latex items: elastic bands, latex paint samples, rubber-backed rugs, exercise bands, some shoe soles.
  • Artwork, photographs, silk or dried flowers, leather goods and antiques.
  • Records, tapes, film and delicate electronics you can carry out.
  • Open cosmetics, essential oils and anything already fragranced.

Access and utilities

  • Leave a key, lockbox code or someone to let us in, and confirm the alarm code.
  • Leave power on to the property.
  • Point out the thermostat and HVAC returns; tell us if the system should run.
  • Tell us about smoke or CO detectors that need covering, and about any sprinkler heads.
  • Close and lock exterior windows and doors; let neighbors or attached units know if air is shared.
  • Note anything valuable, fragile or off-limits before we start.

Safety questions

These keep everyone safe on treatment day. Answer every one — a “heads up” answer does not stop the job, it just tells us how to plan it.

  • Does anyone at the property have asthma, COPD, another breathing or heart condition, or is anyone pregnant, an infant or elderly?

We can still treat — we just plan longer clear-out and aeration time.

  • Is anyone at the property using oxygen or other home medical equipment?

  • Are there pets, birds, fish, reptiles or other animals at the property?

Birds and fish are especially sensitive and must leave the building, not just the room.

  • Are there live plants in or near the area being treated?

  • Can everyone — people, pets and plants — be out of the treated area for the whole treatment and until we say the air has cleared?

  • Is the space attached to another home, apartment or business that shares air, a hallway or ductwork?

  • Are there rubber or latex items, artwork, photographs, silk plants, leather, electronics, medications or open food that need to be removed first?

  • Will all food, drink and medication be sealed or removed from the treated area?

  • Will we have access to the HVAC system, smoke alarms and a key or lockbox code?

  • Is anyone at the property sensitive or allergic to cleaning chemicals, disinfectants, fragrances or strong scents?

We can switch to a low-odor or fragrance-free product, or use hydroxyl instead of fogging.

  • Will anyone need to stay in the building while we treat (for example an attached unit, a business next door or an overnight occupant)?

If so we use hydroxyl and air scrubbing instead of ozone.

  • Are there kitchen counters, dishes, cookware, toys or pet bowls in the area we are treating?

Anything that touches food, a child or a pet gets covered, removed or washed after treatment.

  • Is there an aquarium, terrarium or beehive that cannot be moved?

  • Are there antiques, musical instruments, specialty finishes, natural stone or artwork we should protect from chemicals or fogging?

  • Would you like the Safety Data Sheet (SDS) for the products we use emailed to you?

Either answer is fine — you can request them at any time.

  • Will you (owner or tenant) have the area cleaned and prepped — belongings, laundry, trash, food, dishes and litter boxes cleared — before we arrive?

If you would rather we do the prep cleaning, answer No and we will quote it as an added charge.

  • Do you understand nobody may re-enter until we confirm the space is clear?
